Your SHN mentor provides support, advice, clinical expertise, and implementation strategies. The mentor will enhance the capacity of your healthcare team to successfully implement the MedRec intervention, and support and advance shared learning.

To request a mentor for the MedRec intervention:
Read mentor profiles posted on the MedRec Community of Practice.
Select a mentor organization that you believe will help you achieve your goals.
Contact the intervention lead listed in the selected profile.
The intervention lead will contact the mentor organization you have requested to determine availability. Upon confirmation of availability, the intervention lead will connect you to the mentor organization.
