The Institute for Safe Medication Practices (ISMP) Canada is an independent national non-profit agency established for the collection and analysis of medication error reports and the development of recommendations for the enhancement of patient safety. It serves as a national resource for promoting safe medication practices throughout Canada’s healthcare community.
The vision of ISMP Canada is to be part of an international network that shares recommendations for the prevention of error-induced patient injuries relating to medication. ISMP Canada is committed to the safe use of medications through improvement in drug distribution (institution and community), naming, packaging, labelling, computer program design, and drug delivery system design. The Institute collaborates with others to provide education about adverse drug events and their prevention.
ISMP Canada was delighted to lead the “Canadianizing” of the Getting Started Kits for the medication reconciliation interventions of Safer Healthcare Now!. In addition, ISMP Canada is the lead for the medication reconciliation interventions, providing support to teams as they make the complex system changes that medication reconciliation requires.
