Getting Started With Medication Reconciliation in Long Term Care / En avant avec le bilan comparatif des médicaments en soins de longue durée- Les bases et plus

Purpose of the call:

The following SHN national medication reconciliation call is geared to participants who are
getting started with medication reconciliation in long term care. At the completion of the call, participants will understand the process of medication reconciliation, what to measure, how to join the SHN campaign and access resources on the communities of practice.  Participants will also learn from experienced practitioners who will share their journeys of implementing medication reconciliation successfully in their own institutions.

Cet appel national vise les participants qui commencent le bilan comparatif des médicaments (BCM) en soins de longue durée. À la fin de la conférence téléphonique, les participants auront une meilleure compréhension du BCM, savoir quoi mesurer, comment s’inscrire aux SSPSM et avoir accès aux ressources disponibles sur la communauté de pratique.

Des professionnels de la santé partageront aussi leur expérience de mise en oeuvre du BCM.
